Job Description:

We are seeking a passionate and dedicated Registered Nurse (RN) to join our Home Health team for a 13-week travel contract. As an RN in Home Health, you will have the opportunity to provide compassionate care to patients in the comfort of their own homes, helping them maintain their independence and improve their quality of life.

Responsibilities:

Conduct patient assessments and develop individualized care plans
Administer medications and treatments as prescribed by physicians
Monitor patient progress and update care plans accordingly
Educate patients and their families on managing their conditions and promoting wellness
Collaborate with other healthcare professionals to ensure coordinated and comprehensive care
Maintain accurate and up-to-date documentation of patient care activities

Requirements:

Valid RN license in the state of Tennessee (TN)
Minimum of [Specify years] years of nursing experience, preferably in Home Health
Experience with Homecare Home Base software is preferred
Strong communication and interpersonal skills
Ability to work independently and as part of a multidisciplinary team
Reliable transportation and willingness to travel within the designated area
