Job Description

At Sunbelt staffing, we are currently looking for a licensed Nurse Practitioner (NP) to conduct in-home assessments in Alameda, CA and throughout the Alameda County on a per diem basis. The qualified candidate will be responsible for covering MC Advantage Member's annual In-Home Health assessment throughout the county.

Each visit will take about 45- 60 minutes. No Prescribing responsibilities, visits are not meant to replace PCP or existing doctors appointments.

This position offers a great opportunity for a highly motivated and detail-orientated individual looking to work independently with flexible hours with the ability to self-schedule.

Availability can be divided amongst any days between Sunday - Saturday, 8am - 8pm making it possible to fit around most FT schedules. 

Candidate should have 6 months experience employed as an NP and/or Home Health experience/exposure in healthcare career.

Responsibilities:

  • Conduct annual home health risk assessments for a Medicare Advantage health plan.
  • Evaluate prior and current medical history, medical conditions and medication history.
  • Provide education on medical conditions and medications.
  • Identify any gaps in care and provide recommendations for resolution.
  • No creation of treatment plans or prescribing of medications is required.

Preferred Qualifications:

  • Nurse Practitioner License in CA.
  • Current CPR certification (BLS).
  • Board Certification through AANP or ANCC.

Preferred Experience:

  • 6 months of current NP experience is preferred.

Education Requirements:

  • MSN degree from an accredited school of nursing.

Work Hours:

  • Flexible shifts between Monday and Sunday
  • Create your own schedule.
  • Minimum weekly or bi-weekly hourly commitment.
