Job Description

At Sunbelt staffing, we are currently looking for a licensed Nurse Practitioner (NP) to conduct in-home assessments in Alameda, CA and throughout the Alameda County on a per diem basis. The qualified candidate will be responsible for covering MC Advantage Member's annual In-Home Health assessment throughout the county.

Each visit will take about 45- 60 minutes. No Prescribing responsibilities, visits are not meant to replace PCP or existing doctors appointments.

This position offers a great opportunity for a highly motivated and detail-orientated individual looking to work independently with flexible hours with the ability to self-schedule.

Availability can be divided amongst any days between Sunday - Saturday, 8am - 8pm making it possible to fit around most FT schedules.

Candidate should have 6 months experience employed as an NP and/or Home Health experience/exposure in healthcare career.

Responsibilities:

Conduct annual home health risk assessments for a Medicare Advantage health plan.
Evaluate prior and current medical history, medical conditions and medication history.
Provide education on medical conditions and medications.
Identify any gaps in care and provide recommendations for resolution.
No creation of treatment plans or prescribing of medications is required.

Preferred Qualifications:

Nurse Practitioner License in CA.
Current CPR certification (BLS).
Board Certification through AANP or ANCC.

Preferred Experience:

6 months of current NP experience is preferred.

Education Requirements:

MSN degree from an accredited school of nursing.

Work Hours:

Flexible shifts between Monday and Sunday
Create your own schedule.
Minimum weekly or bi-weekly hourly commitment.
