Are you a compassionate and experienced Registered Nurse (RN) seeking a fulfilling full-time position in Jacksonville, Florida? We're actively searching for a dedicated RN to join our team for a permanent role in home health. If you boast at least a year of recent experience in home health, proficiency in OASIS and Homecare Homebase (HCHB), solid clinical skills, and a genuine drive to provide top-notch patient care, we're eager to connect with you!

Your Role:

Collaborate closely with healthcare professionals to design comprehensive home care treatment plans.
Efficiently document and track patient conditions and progress using OASIS and HCHB systems.
Administer medication and offer essential wound care as needed.
Utilize your expertise in OASIS and HCHB for accurate documentation and reporting.

Requirements:

Active RN licensure in Florida.
At least one year of recent home health experience.
Proficiency in OASIS and Homecare Homebase (HCHB) systems.
Current BLS Certification from the American Heart Association.
Previous experience managing a home-to-home workload is advantageous.

Additional Details:

Mileage reimbursement provided for travel within Jacksonville.
