Job Title: Contract Nurse Practitioner (NP)
Employment Type: Full Time, Contract
Duration: 4-7 Months
Location: Multnomah County, Oregon
Coverage: Looking for someone willing to cover 3 SNF facilities across Multnomah County

Overview: We are in search of a committed Nurse Practitioner to perform thorough assessments for patients during short-term stays in a SNF (Skilled Nursing Facility). The responsibilities include working closely with interdisciplinary teams to create personalized care plans and engage in advance care planning for residents. The primary objective of this position is to improve the quality of care, reduce unnecessary hospitalizations, and ensure alignment with the unique care goals of each individual.

Responsibilities:

  • Conduct one-time comprehensive assessments, including physical exams, medication reconciliation, medical history, ADLs, nutritional and cognitive assessments, pain evaluation, fall risk assessment, and social determinants of health evaluation.
  • Develop and communicate problem lists and recommended treatment plans for each resident.
  • Ensure accurate and complete documentation, including ICD10 coding.
  • Collaborate effectively with the interdisciplinary care team.
  • Facilitate advanced illness and advanced care planning conversations.

Requirements:

  • Active NP license in the state of practice, free of any disciplinary actions.
  • Minimum 1 year of NP experience.
  • Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care.
  • Reliable transportation for assessments in nursing homes, assisted living, and home settings.
  • DEA certification.
  • Covid Vaccine

Schedule:
Monday through Friday, 8 am to 5 pm.
