Job Description

RN Supervisor is needed in Oak Park, IL for a full-time contracted position. The ideal candidate would be flexible with their work schedule and have the ability to pass a background check and drug test. Please see more details below: 

Job Responsibilities:  

  • Perform clinical intake of new clients into the program. 

  • Supervise medical assistants, including directing plans of care and supporting MAs in the delivery of quality care.vide ongoing client clinical care coordination with discharging hospitals, pharmacies and primary/specialty care clinics. 

  • Appropriately elevate client clinical concerns with MRC volunteer physicians. 

  • Problem-solving client issues related to access to care. 

  • Assist clients with self-management of medical problems including but not limited to administration of intravenous antibiotics, wound dressing changes, and adherence to prescribed medications. 

  • Ensure proper documentation of client progress in the data management system by monitoring, reporting, and recording changes in clients’ conditions; support medical assistant adherence to documentation standard  

  • Assess, implement plan, and evaluate nursing care plans by working with multidisciplinary team members. Maintain MA work schedules, ensuring 24/7 clinical coverage. 

  • Arrange transportation for clinical appointments. 
    Coordinate telehealth visits. 

  • Maintain adequate clinical and office supplies. 

  • Provide written hand-offs to agencies receiving discharged clients. 

  • Other duties as assigned. 

Job Details: 

  • 8am-4:30pm 

  • Monday-Friday 

  • 40 Hours a week 

  • Contract ending on 12/31 with the possibility of an extension 

  • The position is responsible for supervising and providing clinical care for socially vulnerable adults and families. Routine duties include assessing clinical and non-clinical needs, coordinating care with hospital providers, assisting and working with a multi-disciplinary housing team in a temporary housing setting. 

Minimal Qualifications: 

  • Bachelor’s Degree or diploma in nursing 

  • Current state licensure as a registered nurse 

  • American heart Association BLS certification 

  • Basic computer skills, Microsoft Excel and Word 

  • Familiarity with basic infection control protocols and procedures 

Preferred Qualifications: 

  • Familiarity with Spanish or Polish 

  • Familiarity with home health care 

  • Strong connections with public health as demonstrated through work experiences or collaborations 

  • Working with persons experiencing homelessness, 

  • Understanding of trauma informed care philosophy and practices
