Key Responsibilities:

• Deliver skilled physical therapy treatments according to each plan of care established by the supervising Physical Therapist
• Support patients with therapeutic exercises, gait and balance training, transfer techniques, and mobility tasks
• Closely monitor and document patient responses, reporting significant changes to the supervising PT and care team members
• Provide targeted education to patients and caregivers on fall prevention, assistive device use, and effective home exercise programs
• Maintain detailed, timely documentation using Home Care Home Base (HCHB)
• Work collaboratively with nurses, OTs, SLPs, social workers, and home health aides
• Participate in case conferences and adhere to all related agency and regulatory requirements

Desired Qualifications:

• Active, valid Physical Therapist Assistant (PTA) license in the state of practice
• At least one year of recent Home Health PTA experience, with fieldwork preferred
• Familiarity with Home Care Home Base (HCHB) and OASIS documentation a plus
• Excellent clinical observation, communication, and organizational abilities
• Reliable transportation, valid drivers license, and current auto insurance
• Ability to work independently and efficiently manage schedules for daily travel
