Job Description

Embark on a meaningful career as a Home Health Physical Therapist Assistant (PTA), delivering skilled and compassionate care to patients in their own homes near Virginia Beach, VA. As a contract PTA, youll work closely with a licensed Physical Therapist to implement tailored treatment plans that promote mobility and foster independence. This role offers the opportunity to make a direct, positive impact through hands-on therapy, all while enjoying the flexibility and variety that comes with providing care in a field-based setting.

Key Responsibilities:

  • Carry out individualized physical therapy treatments based on established care plans.
  • Assist patients with therapeutic exercises, gait training, transfers, and other activities designed to enhance functional movement.
  • Carefully monitor, document, and report patient responses and progress toward goals.
  • Reinforce education with patients and caregivers on safety, use of assistive devices, fall prevention, and effective home exercise approaches.
  • Maintain accurate, timely documentation using Home Care Home Base (HCHB).
  • Collaborate with a multidisciplinary teamincluding nurses, occupational therapists, and social workersto ensure comprehensive care.
  • Actively participate in team meetings and case conferences to support coordinated efforts.
  • Uphold agency policies and all regulatory and Medicare standards.

Qualifications:

  • Current, active Physical Therapist Assistant (PTA) license in good standing.
  • At least one year of recent home health PTA experience; fieldwork experience is valued.
  • Proficiency with Home Care Home Base (HCHB) and understanding of OASIS processes preferred.
  • Excellent clinical observation, communication, and organizational skills.
  • Reliable transportation with valid drivers license and insurance.
  • Self-starter, able to manage a dynamic field-based caseload.
