In this contract position, youll deliver skilled therapy services to individuals in their homes throughout the Towson, MD area, working directly under the supervision of a licensed Physical Therapist. What youll do:

  • Implement therapeutic interventions based on individualized care plans to boost mobility, strength, and independence
  • Assist patients with exercises, gait and balance activities, transfers, and functional tasks
  • Monitor patient progress, documenting outcomes and changes in condition using Home Care Home Base (HCHB)
  • Keep the supervising PT and interdisciplinary care team updated on patient status
  • Educate patients and caregivers about exercises, safety measures, assistive equipment, and fall prevention
  • Participate in case conferences, maintaining timely, thorough documentation
  • Comply with all agency processes, regulatory standards, and Medicare guidelines

What were looking for:

  • Active PTA license in good standing within Maryland
  • Minimum of one year of recent experience in home health (field-based experience preferred)
  • Comfortable using Home Care Home Base (HCHB); OASIS knowledge a plus
  • Skill in clinical observation and strong communication
  • Dependable transportation, valid drivers license, and current auto insurance
  • Self-driven, detail-oriented, and able to manage a mobile schedule
