Step into a rewarding role as a Home Health Physical Therapist Assistant (PTA), providing crucial therapy services directly in patients homes near Tallahassee, FL. As a key member of the interdisciplinary care team, youll help empower individuals to regain mobility, strength, and confidence by implementing tailored treatment plans under the supervision of a licensed Physical Therapist.

In this contract position, youll work independently throughout your assigned territory, managing a caseload of approximately 2532 points per week (typically 57 patient visits per day). Your responsibilities will include facilitating therapeutic exercises, gait and balance training, and transfer support, while closely monitoring and documenting patient progress through an electronic documentation system (Home Care Home Base - HCHB). Youll also play an important role in educating patients and caregivers, supporting a safe and effective home recovery.

What youll bring to the table:

• Active PTA license in the state of practice (in good standing)
• Minimum 1 year of recent home health experience as a PTA (field-based experience preferred)
• Familiarity with Home Care Home Base (HCHB) and/or OASIS documentation (preferred)
• Strong clinical assessment and communication skills
• Proven ability to work independently and manage a flexible, field-based schedule
• Reliable transportation, valid drivers license, and auto insurance
