What Youll Do:

• Carry out skilled physical therapy treatments following individualized care plans.
• Guide patients through exercises, gait and balance training, and functional mobility activities.
• Track, document, and communicate patient responses, collaborating closely with a supervising Physical Therapist.
• Empower patients and caregivers with education on safety, fall prevention, and home exercise routines.
• Maintain timely, accurate documentation using Home Care Home Base (HCHB).
• Collaborate with an interdisciplinary team and participate in care conferences.
• Safeguard compliance with agency policies and Medicare guidelines.
• Travel within your assigned territory, with mileage reimbursement.

Your Background:

• Licensed as a Physical Therapist Assistant (PTA) in good standing.
• At least 1 year of recent home health experience required; field experience preferred.
• Familiarity with HCHB and OASIS documentation preferred.
• Excellent clinical, communication, and time management skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to thrive autonomously and adapt to a flexible schedule.
