In this position, you'll employ your therapy expertise during weekday visits, working with patients on exercises, assistive techniques, and strategies to enhance safety and functional mobility. Your keen clinical judgment and documentation skills will be vital for tracking patient progress and ensuring the highest standards of care throughout the therapeutic journey.
Key Qualifications:
- Valid PTA license in good standing in the state of practice
- At least 1 year of recent Home Health PTA experience preferred
- Familiarity with Home Care Home Base (HCHB) electronic documentation a plus
- Understanding of OASIS documentation processes is helpful
- Reliable vehicle, valid drivers license, and current auto insurance
- Excellent communication, interpersonal, and time management skills
- Ability to work efficiently and independently in a flexible, mobile environment
Role Highlights:

- Guide patients through therapeutic exercises, gait, and transfer training
- Educate and support patients and caregivers on safety, independence, and recovery
- Maintain clear, timely documentation in HCHB
