In this contract role, youll deliver skilled therapeutic services under the guidance of a licensed Physical Therapist, combining your clinical skills with the ability to work autonomously in a dynamic, field-based setting.
In this position, you'll engage directly with patients, implementing individualized treatment plans tailored to each persons unique needs. Your day-to-day will include not only hands-on therapy but also crucial communication with both the supervising therapist and multidisciplinary team, ensuring every patient receives the highest standard of care.
Key Qualifications:
- Active Physical Therapist Assistant (PTA) license in good standing
- At least one year of recent experience in home health physical therapy (field experience highly valued)
- Familiarity with electronic documentation systems, specifically Home Care Home Base (HCHB), preferred
- Understanding of OASIS processes is a plus
- Strong observation, assessment, and communication abilities
- Reliable transportation, valid drivers license, and current auto insurance
- Capacity to work independently, prioritize tasks, and manage a flexible field-based schedule
Primary Responsibilities:
- Administer skilled therapy interventions per the plan of care
- Guide patients through therapeutic exercises, mobility activities, and safety strategies
- Monitor and document patient progress and update the supervising PT on changes
- Educate patients and caregivers on exercise regimens and fall prevention
- Ensure all records are completed accurately and on time within HCHB
- Collaborate actively with the full care team and participate in team meetings
