Your day-to-day will include not only hands-on therapy but also crucial communication with both the supervising therapist and multidisciplinary team, ensuring every patient receives the highest standard of care.

Key Qualifications:

• Active Physical Therapist Assistant (PTA) license in good standing
• At least one year of recent experience in home health physical therapy (field experience highly valued)
• Familiarity with electronic documentation systems, specifically Home Care Home Base (HCHB), preferred
• Understanding of OASIS processes is a plus
• Strong observation, assessment, and communication abilities
• Reliable transportation, valid drivers license, and current auto insurance
• Capacity to work independently, prioritize tasks, and manage a flexible field-based schedule

Primary Responsibilities:

• Administer skilled therapy interventions per the plan of care
• Guide patients through therapeutic exercises, mobility activities, and safety strategies
• Monitor and document patient progress and update the supervising PT on changes
• Educate patients and caregivers on exercise regimens and fall prevention
• Ensure all records are completed accurately and on time within HCHB
• Collaborate actively with the full care team and participate in team meetings
