Job Description

Bring your clinical expertise and compassion for patient care to a rewarding contract opportunity as a Home Health Physical Therapist Assistant (PTA). In this full-time, field-based role, youll deliver skilled therapy services directly to patients in their homes, working collaboratively under the direction and supervision of a licensed Physical Therapist. Your ability to foster independence, improve mobility, and encourage patient success will be key as you help individuals achieve their rehabilitation goals.

Key Responsibilities:

  • Provide hands-on physical therapy interventions aligned with the treatment plan developed by the supervising Physical Therapist.
  • Assist patients with therapeutic exercises, gait and balance training, transfer techniques, and functional mobility tasks.
  • Observe, monitor, and document patient progress toward established goals using Home Care Home Base (HCHB).
  • Communicate observations, changes in patient condition, and progress to the supervising PT and care team.
  • Offer education to patients and caregivers on safety, assistive devices, fall prevention, and exercise routines.
  • Maintain accurate and timely electronic documentation.
  • Collaborate with interdisciplinary team members, engaging in case conferences as needed.
  • Follow agency policies and adhere to all healthcare standards and guidelines.

Qualifications:

  • Active Physical Therapist Assistant (PTA) license in good standing in the state of practice.
  • At least one year of recent experience as a PTA in a home health setting.
  • Familiarity with HCHB preferred and understanding of OASIS documentation a plus.
  • Reliable transportation, valid drivers license, and current auto insurance.
  • Strong communication, organizational, and time management skills.
  • Comfortable working independently and managing a flexible schedule.
