What Youll Do:
- Deliver individualized physical therapy interventions as outlined by a licensed Physical Therapist
- Support patients through therapeutic exercises, gait and balance training, transfer assistance, and functional activities
- Monitor and document progress, reporting changes and collaborating on care plan adjustments
- Provide education to patients and caregivers on exercises, assistive device use, fall prevention, and safety strategies
- Accurately complete electronic documentation using Home Care Home Base (HCHB)
- Collaborate with an interdisciplinary teamnurses, OTs, SLPs, social workers, and home health aides
- Attend team meetings and participate in case conferences as needed
- Adhere to all regulatory guidelines and agency standards, including Medicare home health requirements
Who You Are:
- Possess an active and current Physical Therapist Assistant license for the state
- Have at least one year of recent field-based home health experience as a PTA
- Preferably proficient with Home Care Home Base (HCHB) and familiar with OASIS documentation
- Adaptable, highly organized, and capable of working independently within a flexible territory-based schedule
- Skilled in clinical observation, communication, and patient instruction
- Equipped with reliable transportation, a valid drivers license, and auto insurance
