Job Description

Make a meaningful impact as a Home Health Physical Therapist Assistant, helping patients regain independence and reach their functional goalsall within the comfort of their own homes. This contract opportunity offers the chance to provide personalized care while enjoying the benefits of a flexible, field-based setting near Phoenix, AZ.

In this role, youll implement physical therapy interventions under the guidance of a licensed Physical Therapist, traveling to patient homes within your assigned territory. You'll play a key role in each patients recovery by delivering high-quality care, monitoring progress, and fostering supportive relationships with patients and caregivers.

Key Responsibilities:

  • Deliver skilled physical therapy treatments per individualized care plans
  • Guide patients through therapeutic exercises, gait and balance training, and functional mobility tasks
  • Track and document patient responses and progress using Home Care Home Base (HCHB)
  • Report patient changes and collaborate closely with the supervising Physical Therapist and care team
  • Reinforce education on safety, assistive devices, fall prevention, and home exercise programs
  • Ensure timely, accurate documentation and maintain compliance with all regulatory standards

Desired Qualifications:

  • Active PTA license (state of practice, in good standing)
  • Minimum 1 year recent Home Health PTA experience; field experience preferred
  • Experience with Home Care Home Base (HCHB) highly desirable
  • Familiarity with OASIS documentation processes preferred
  • Clear communication, strong clinical assessment, and independent time management
  • Reliable transportation, valid drivers license, and auto insurance
