Seeking a dedicated Home Health Physical Therapist Assistant to deliver compassionate, high-quality therapy care to patients in their homes near Norfolk, VA. In this impactful field-based role, you will collaborate closely with supervising Physical Therapists to implement individualized treatment plans, enhance functional independence, and support ongoing recoveryall while working independently within a supportive team environment.
Your responsibilities will include:
- Administering skilled physical therapy interventions according to the established plan of care
- Guiding patients through therapeutic exercises, gait and balance training, and mobility activities
- Monitoring and clearly documenting patient progress using the Home Care Home Base (HCHB) electronic system
- Proactively updating the supervising Physical Therapist and interdisciplinary team about patient status or any changes observed
- Educating patients and caregivers on safe mobility, fall prevention, proper use of assistive devices, and home exercise programs
- Participating in case conferences and collaborating with the broader healthcare team
- Adhering to agency policies, regulatory standards, and Medicare home health guidelines
Qualifications were looking for:
- Current, active PTA license in good standing for the state of practice
- At least 1 year of recent home health PTA experience (field-based work preferred)
- Familiarity with Home Care Home Base (HCHB) and OASIS documentation strongly preferred
- Excellent clinical observation, time management, and communication abilities
- Valid drivers license, reliable transportation, and active auto insurance
- Organized, self-motivated, and confident working both independently and as part of a team
