Key Qualifications:

• Current and active PTA license in the state
• At least one year of recent experience as a Home Health PTA (field-based preferred)
• Proficient with Home Care Home Base (HCHB) electronic documentation (preferred)
• Familiarity with OASIS documentation is a plus
• Reliable personal transportation, a valid drivers license, and proof of auto insurance
• Outstanding clinical judgment, observation, and interpersonal communication skills
• Ability to self-manage a flexible caseload in patients home environments

Core Responsibilities:

• Deliver skilled physical therapy services under the supervision of a Physical Therapist
• Assist patients with therapeutic exercises, gait training, and balance and transfer activities
• Monitor, document, and report patient progress and any changes in condition
• Educate patients and caregivers on safety protocols, home exercise, and use of assistive devices
• Accurately complete documentation in HCHB
• Collaborate with an interdisciplinary home health team to facilitate optimal outcomes
• Participate in meetings and ensure compliance with agency procedures and Medicare guidelines
