Key Qualifications:
- Current and active PTA license in the state
- At least one year of recent experience as a Home Health PTA (field-based preferred)
- Proficient with Home Care Home Base (HCHB) electronic documentation (preferred)
- Familiarity with OASIS documentation is a plus
- Reliable personal transportation, a valid drivers license, and proof of auto insurance
- Outstanding clinical judgment, observation, and interpersonal communication skills
- Ability to self-manage a flexible caseload in patients home environments
Core Responsibilities:
- Deliver skilled physical therapy services under the supervision of a Physical Therapist
- Assist patients with therapeutic exercises, gait training, and balance and transfer activities
- Monitor, document, and report patient progress and any changes in condition
- Educate patients and caregivers on safety protocols, home exercise, and use of assistive devices
- Accurately complete documentation in HCHB
- Collaborate with an interdisciplinary home health team to facilitate optimal outcomes
- Participate in meetings and ensure compliance with agency procedures and Medicare guidelines
