A rewarding opportunity awaits for a skilled and compassionate Home Health Physical Therapist Assistant (PTA). In this contract role, youll work one-on-one with patients in their homes, delivering high-quality therapy services under the guidance of a licensed Physical Therapist. Your work will support patients journeys toward greater mobility, strength, independence, and quality of life, while you enjoy autonomy and the chance to make a direct impact in the community.
This position is ideal for a motivated, organized PTA who thrives in a field-based environment and enjoys building meaningful connections with patients and caregivers. Youll carry out therapeutic interventions, reinforce education for continued recovery at home, and collaborate closely with a multidisciplinary care team. Expect a manageable caseload of roughly 2532 points per week, typically involving 57 visits daily, Monday through Friday, 8:00 AM to 5:00 PM.
Responsibilities:
- Deliver prescribed physical therapy treatments in accordance with each patients individualized plan of care.
- Guide patients through therapeutic exercises, mobility training, and functional tasks.
- Monitor and document responses and progress using Home Care Home Base (HCHB).
- Communicate patient status updates to the supervising PT and team.
- Educate patients and caregivers about safe movement, assistive devices, and home programs.
- Participate in case conferences and adhere to regulatory standards, including Medicare guidelines.
Preferred Qualifications:
- Active PTA license (in good standing) in the state of practice.
- At least 1 year of recent field-based PTA experience, ideally in home health.
- Proficiency in HCHB and understanding of OASIS documentation.
- Strong clinical, communication, and organizational skills.
- Reliable vehicle, current drivers license, and insurance.
