Key Responsibilities:

  • Provide evidence-based physical therapy treatments according to individualized care plans developed by a supervising PT.
  • Support patients with therapeutic exercises, balance activities, gait and transfer training, and functional mobility tasks.
  • Monitor and document patient responses, updating care plans and reporting changes to the supervising Physical Therapist.
  • Deliver patient and caregiver education on safety strategies, use of assistive devices, and home exercise regimens to support lasting recovery.
  • Maintain accurate, timely documentation using Home Care Home Base (HCHB) software.
  • Collaborate with nurses, occupational therapists, speech-language pathologists, social workers, and home health aides to ensure holistic patient care.
  • Attend team meetings and participate in case conferences as needed.
  • Uphold compliance with agency policies, state and federal regulations, and Medicare home health guidelines.

Qualifications:

  • Active Physical Therapist Assistant (PTA) license in good standing within the state of practice.
  • Minimum of 1 year recent Home Health PTA experience (field-based experience highly valued).
  • Familiarity with Home Care Home Base (HCHB) and OASIS documentation is preferred.
  • Outstanding observational, communication, time management, and organization skills.
  • Reliable vehicle, valid drivers license, and auto insurance.
