Job Description

Make a meaningful difference in the lives of patients as a dedicated Home Health Physical Therapist Assistant (PTA), delivering essential therapy services directly in patients homes. Working independently yet collaboratively under the direction of a licensed Physical Therapist, you will help individuals enhance their mobility, strength, and independence through individualized care.

Key Responsibilities:

  • Provide skilled physical therapy interventions according to each patients established treatment plan
  • Assist patients with therapeutic exercises, gait, balance, transfers, and functional mobility
  • Monitor and document patient progress, reporting changes to supervising Physical Therapist and team members
  • Reinforce patient and caregiver education on safety, home exercise programs, assistive devices, and fall prevention
  • Maintain accurate, timely documentation using Home Care Home Base (HCHB)
  • Collaborate regularly with the interdisciplinary healthcare team and participate in team meetings
  • Travel daily to patient homes within your assigned territory (with mileage reimbursement)

Requirements:

  • Active PTA license in good standing in the state of practice
  • At least 1 year of recent Home Health PTA experience (field-based experience preferred)
  • Experience with Home Care Home Base (HCHB) preferred
  • Familiarity with OASIS documentation a plus
  • Strong clinical and communication skills
  • Reliable transportation, valid driver's license, and auto insurance
  • Excellent organization, time management, and the ability to work independently
