We are searching for a dedicated Home Health Physical Therapist Assistant (PTA) to deliver skilled therapy services in a field-based setting. Youll have the unique opportunity to help individuals regain independence, working collaboratively with a supervising Physical Therapist while providing one-on-one care in the comfort of each patients home.
This full-time, 13-week contract runs Monday through Friday, 8:00 AM to 5:00 PM, serving patients throughout your assigned territory. Expect a caseload that allows you to focus on individualized caretypically 57 visits per day, depending on visit type. With mileage reimbursement and use of the Home Care Home Base (HCHB) electronic documentation system, youll be supported as you travel to patient homes.
Desired Qualifications:
- Current, active PTA license (in good standing) for the state of practice
- At least one year of recent Home Health PTA experience preferred
- Familiarity with Home Care Home Base (HCHB) electronic documentation strongly desired
- Understanding of OASIS processes is a plus
- Reliable transportation, valid drivers license, and auto insurance
- Excellent communication, observation, and time management skills
- Ability to work independently, managing a flexible schedule
Key Responsibilities:
- Provide skilled PTA interventions guided by individualized care plans
- Facilitate therapeutic exercises, gait and balance training, and safe functional mobility
- Monitor, document, and communicate patient progress and any changes to the care team
- Reinforce education for patients and caregivers, emphasizing safety and home exercise programs
- Maintain thorough and timely records in HCHB
- Collaborate actively with an interdisciplinary team during case conferences
