What were looking for:
- Active Physical Therapist Assistant (PTA) license in the applicable state (in good standing)
- Minimum of 1 year of recent Home Health PTA experience
- Familiarity with Home Care Home Base (HCHB) and OASIS documentation processes preferred
- Strong clinical assessment, observation, and communication skills
- Ability to work independently and efficiently manage a field-based schedule
- Reliable transportation, valid drivers license, and appropriate auto insurance
- Excellent time management and organizational abilities
Role responsibilities:
- Administer skilled physical therapy interventions following established plans of care
- Guide patients through exercises, gait and balance training, transfer techniques, and daily mobility tasks
- Carefully track patient progress and document responses in HCHB
- Communicate patient status changes and updates to the supervising PT and care team
- Reinforce home safety strategies, proper use of assistive devices, and home exercise programs
- Collaborate with healthcare professionals across disciplines in team meetings and case conferences
- Uphold agency protocols, regulatory standards, and Medicare guidelines
