Job Description

Make a meaningful impact by assisting patients in their recovery journeys as a Home Health Physical Therapist Assistant (PTA). In this vital field-based contract role, youll bring skilled physical therapy services directly to patients in their homes. Under the supervision of a licensed Physical Therapist, you will implement individualized care plans focused on improving mobility, strength, balance, and function, helping individuals regain independence and enhance quality of life.

What Youll Do:

  • Deliver evidence-based physical therapy interventions according to established plans of care.
  • Guide patients through therapeutic exercises, transfer training, gait, balance, and functional mobility activities.
  • Observe and record patient progress, updating the supervising PT and care team on changes or milestones.
  • Provide clear education to patients and caregivers on home exercise programs, safety, fall prevention, and use of assistive devices.
  • Collaborate with a diverse home health team, including nurses, occupational and speech therapists, social workers, and aides.
  • Maintain comprehensive and timely documentation using Home Care Home Base (HCHB).
  • Travel daily to patient homes within your assigned area (mileage reimbursement included).

Qualifications & Experience:

  • Active PTA license in good standing for the state of assignment.
  • Minimum one year of recent experience as a Home Health PTA.
  • Familiarity with Home Care Home Base (HCHB) and OASIS documentation preferred.
  • Strong clinical, communication, and organizational skills.
  • Ability to work independently and manage a flexible caseload (MondayFriday, 8:00 AM5:00 PM).
  • Reliable transportation, valid drivers license, and auto insurance.
