This contract position offers you the autonomy to deliver skilled physical therapy services to individuals in their homes, under the guidance of a licensed Physical Therapist. You'll make a meaningful impact by helping patients regain mobility, strength, and independence through personalized rehabilitation plans.
Key Responsibilities:
- Deliver hands-on, skilled therapy treatments following the care plan established by the supervising Physical Therapist.
- Support patients with therapeutic exercises, gait and balance training, transfer techniques, and functional mobility activities.
- Accurately monitor, document, and report patient progress using Home Care Home Base (HCHB).
- Communicate any changes in patient status promptly to the supervising PT and care team.
- Provide clear patient and caregiver education on safety, equipment use, home exercise routines, and fall prevention.
- Participate actively in interdisciplinary team meetings, case conferences, and collaborative care planning.
- Maintain compliance with agency protocols and home health regulations.
- Travel locally to patient residences, with mileage reimbursement included.
Desired Qualifications:
- Active, in-good-standing PTA license in the state of practice.
- At least 1 year of recent field-based Home Health PTA experience strongly preferred.
- Experience documenting in HCHB; knowledge of OASIS documentation is a plus.
- Dependable transportation, a valid drivers license, and current auto insurance.
- Excellent observation, communication, and organizational skills.
- Ability to work independently and flexibly in a dynamic home health environment.
