As a valued Home Health PTA, youll follow defined care plans alongside a licensed Physical Therapist, assisting patients with personalized exercises, mobility training, and rehabilitation techniques. Your ability to educate patients and caregivers on safe, effective home routines will help foster lasting independence and well-being.

Key Responsibilities:

• Deliver skilled physical therapy interventions following the PTs care plan
• Guide patients through exercises, balance activities, gait and transfer training
• Document progress using Home Care Home Base (HCHB)
• Communicate changes or concerns to the PT and broader care team
• Provide patient/caregiver education about safety and home programs
• Participate in interdisciplinary collaboration, team meetings, and case discussions
• Maintain compliance with agency policies and regulatory standards

Preferred Qualifications:

• Active state licensure as a Physical Therapist Assistant
• At least 1 year of recent home health PTA experience
• Proficiency with electronic documentation (HCHB preferred)
• Familiarity with OASIS processes
• Strong verbal and written communication skills
• Reliable vehicle, valid drivers license, and auto insurance
• Ability to work independently and manage time effectively
