Job Description

Working independently in a field-based setting, youll bring personalized therapy directly to patients, utilizing your skills to implement evidence-based treatment plans. Your dedication and communication will ensure each patient receives high-quality, individualized care, while your initiative supports ongoing recovery and functional improvement.

What Youll Do:

  • Deliver skilled physical therapy interventions based on the supervising Physical Therapists established plan of care.
  • Guide patients through therapeutic exercises, gait and balance activities, transfer training, and mobility enhancement.
  • Monitor and document patient progress, updating supervisors and team members to promote best outcomes.
  • Educate patients and caregivers on safety, assistive device use, fall prevention, and home exercise routines.
  • Complete prompt and thorough electronic documentation using Home Care Home Base (HCHB).
  • Collaborate with interdisciplinary professionalsnurses, rehab team members, and social workersto coordinate care.
  • Attend case conferences and contribute to team meetings.
  • Maintain compliance with agency protocols and home health regulations.

What Were Looking For:

  • Active PTA license in good standing for the state of practice.
  • Minimum 1 year of recent home health PTA experience.
  • Comfortable with HCHB and, ideally, OASIS documentation.
  • Strong clinical evaluation and communication skills.
  • Reliable transportation, valid drivers license, and active auto insurance.
  • Independent work ethic and organizational savvy.
