Job Description

Make a meaningful impact each day as a Home Health Physical Therapist Assistant (PTA), delivering high-quality care directly to patients in the comfort of their own homes. In this 13-week, full-time contract position, youll work Monday through Friday from 8:00 AM to 5:00 PM, serving patients across your assigned territory and helping improve their mobility, independence, and quality of life.

Your expertise will be put to use as you administer skilled therapeutic treatments, implement individualized plans of care designed by a supervising Physical Therapist, and reinforce patient and caregiver education. Youll manage a caseload of approximately 2532 points per week, usually seeing 57 patients daily, and will utilize Home Care Home Base (HCHB) for timely and accurate documentation. Daily travel between patient homes is a key part of this role, and mileage reimbursement is provided.

Qualifications:

  • Active Physical Therapist Assistant (PTA) license in good standing for the state of practice
  • At least 1 year of recent home health PTA experience
  • Proficiency with Home Care Home Base (HCHB) strongly preferred
  • Familiarity with OASIS documentation processes is a plus
  • Excellent clinical observation, organizational, and time management abilities
  • Reliable transportation, valid drivers license, and current auto insurance
  • Strong communication skills and ability to work independently in a field-based setting

Responsibilities:

  • Deliver skilled physical therapy interventions according to established plans of care
  • Assist with exercises, gait, balance, transfer training, and other functional activities
  • Document patient progress and communicate updates to the supervising Physical Therapist
  • Reinforce education on safety, assistive devices, fall prevention, and home exercise routines
  • Collaborate with an interdisciplinary home health team
  • Uphold compliance with agency policies and relevant regulatory guidelines
