Key Responsibilities:

• Deliver skilled physical therapy treatments under the guidance of a licensed Physical Therapist, adapting interventions to each patients needs.
• Facilitate therapeutic exercises, gait and balance training, and mobility tasks to support patient goals.
• Precise documentation and timely updates using Home Care Home Base (HCHB).
• Frequent communication with the care team to report on patient status, ensuring continuity of care.
• Educate patients and caregivers on safety measures, fall prevention, proper use of assistive devices, and ongoing home exercise routines.
• Attend case conferences and collaborate with nurses, occupational therapists, speech-language pathologists, and social workers, as needed.
• Uphold regulatory standards, agency protocols, and Medicare guidelines.

Desired Qualifications:

• Active Physical Therapist Assistant (PTA) license in good standing.
• At least one year of recent home health field experience.
• Familiarity with Home Care Home Base (HCHB) and understanding of OASIS documentation is preferred.
• Excellent clinical judgment and strong communication skills.
• Independent, organized, and confident managing a field-based schedule.
• Reliable transportation, valid drivers license, and auto insurance.
