Embark on a rewarding journey as a Home Health Physical Therapist Assistant (PTA) delivering exceptional care in a field-based setting near Chesapeake, VA. In this full-time, 13-week contract role, youll empower patients to regain independence by providing hands-on therapy under the supervision of a licensed Physical Therapist. Your compassionate approach, expertise, and dedication will help individuals reach their optimal mobility and strengthright in the comfort of their own homes.
Your day-to-day will involve collaborating closely with an interdisciplinary team, supporting a caseload of approximately 2532 points per week (57 patient visits daily). Youll assess and assist patients with therapeutic exercises, gait and transfer training, and design strategies that promote safety and encourage ongoing progress. Daily documentation is completed using Home Care Home Base (HCHB), ensuring continuity and high-quality care.
What Were Looking For:
- Active PTA license in the state of practice, in good standing
- At least 1 year of recent experience as a Home Health PTA, with field work preferred
- Familiarity with Home Care Home Base (HCHB) and OASIS documentation preferred
- Strong clinical observation skills and effective communication
- Reliable transportation, valid drivers license, and current auto insurance
- Self-motivated, organized, and comfortable working independently
Key Responsibilities:
- Implement and monitor individualized therapy plans
- Assist with balance, mobility, and strength-building exercises
- Accurately document patient progress and communicate updates to the supervising PT
- Educate patients and caregivers on safety strategies and home exercises
- Collaborate in case conferences and adhere to Medicare and agency standards
