Bring your expertise and compassion to home health care as a Physical Therapist Assistant, working directly with patients in their homes to help restore strength, mobility, and independence. This contract assignment offers the opportunity to support individuals throughout your assigned territory near Chesapeake, VA, providing rewarding, patient-centered care while maintaining autonomy in your daily schedule.
Key Responsibilities:
- Deliver skilled therapy interventions based on the individualized care plan developed by the supervising Physical Therapist
- Assist patients with therapeutic exercises, gait training, balance activities, and transfer/functional mobility
- Monitor and document patient responses, reporting changes in condition and progress promptly
- Reinforce safety strategies, home exercise programs, assistive device use, and fall prevention with patients and caregivers
- Maintain accurate, timely records using the Home Care Home Base (HCHB) documentation system
- Collaborate with an interdisciplinary team, including participation in meetings and case conferences as required
- Uphold all agency, regulatory, and Medicare guidelines for home health care
Desired Qualifications:
- Active, in-good-standing Physical Therapist Assistant (PTA) license in Virginia
- At least 1 year of recent Home Health PTA experience (field work preferred)
- Familiarity with Home Care Home Base (HCHB) and OASIS processes a plus
- Excellent clinical assessment, communication, and organizational abilities
- Self-motivated with strong time management skills and a professional bedside manner
- Reliable vehicle, valid drivers license, and up-to-date auto insurance
