Job Description

Make a meaningful impact by delivering personalized care as a Home Health Physical Therapist Assistant (PTA) in a field-based setting. This contract position offers the chance to work closely with patients in their homes, facilitating rehabilitation and supporting functional independence under the guidance of a licensed Physical Therapist. Your days will be varied and rewarding as you implement individualized treatment plans, provide therapeutic interventions, and help patients reach their goals through consistent progress monitoring and education.

This role is structured as a full-time, 13-week contract running Monday through Friday from 8:00 AM to 5:00 PM. Expect to complete approximately 2532 points per week, visiting five to seven patients daily, and utilizing the Home Care Home Base (HCHB) documentation system. Daily travel to homes within your assigned area is requiredmileage reimbursement is provided. Youll thrive if you are self-directed, organized, and comfortable with independent work, alongside engaging collaboratively with a wider healthcare team.

Qualifications:

  • Active and unencumbered Physical Therapist Assistant (PTA) license in the practicing state
  • At least one year of recent experience in Home Health physical therapy (field-based preferred)
  • Knowledge of Home Care Home Base (HCHB) and OASIS documentation is highly desirable
  • Proficient in clinical observation, documentation, and reporting patient progress
  • Reliable transportation, valid drivers license, and auto insurance required

Primary Responsibilities:

  • Deliver high-quality physical therapy interventions as outlined by the supervising PT
  • Support patients with exercises, safety instruction, and the use of assistive devices
  • Monitor and communicate patient outcomes to the care team
  • Collaborate with other healthcare professionals and participate in team meetings
