Key Responsibilities:

• Administer skilled physical therapy interventions per individualized care plans developed by a supervising PT
• Facilitate therapeutic exercises, balance and gait training, transfer assistance, and mobility enhancement
• Document patient responses and measurable progress toward functional goals using Home Care Home Base (HCHB)
• Communicate status updates and changes to the care team and supervising PT, contributing to robust care coordination
• Reinforce patient and caregiver education on safety, fall prevention, assistive device use, and home exercise instruction
• Travel to patient homes within your assigned area; mileage reimbursement provided
• Collaborate with an interdisciplinary team to ensure holistic, patient-centered outcomes
• Participate in meetings, case conferences, and comply with all regulatory and agency standards

Qualifications:

• Active Physical Therapist Assistant license (in good standing) in the practicing state
• Minimum of one year of recent home health experience (field-based background preferred)
• Comfortable with Home Care Home Base (HCHB); OASIS documentation familiarity preferred
• Strong assessment, organizational, and communication skills
• Reliable vehicle, valid drivers license, and proof of auto insurance
• Ability to manage a flexible field-based schedule independently
