This dynamic, field-based role involves working closely under the guidance of a licensed Physical Therapist to help patients regain strength, independence, and mobility. Key Responsibilities:
- Administer skilled physical therapy interventions per individualized care plans developed by a supervising PT
- Facilitate therapeutic exercises, balance and gait training, transfer assistance, and mobility enhancement
- Document patient responses and measurable progress toward functional goals using Home Care Home Base (HCHB)
- Communicate status updates and changes to the care team and supervising PT, contributing to robust care coordination
- Reinforce patient and caregiver education on safety, fall prevention, assistive device use, and home exercise instruction
- Travel to patient homes within your assigned area; mileage reimbursement provided
- Collaborate with an interdisciplinary team to ensure holistic, patient-centered outcomes
- Participate in meetings, case conferences, and comply with all regulatory and agency standards
Qualifications:
- Active Physical Therapist Assistant license (in good standing) in the practicing state
- Minimum of one year of recent home health experience (field-based background preferred)
- Comfortable with Home Care Home Base (HCHB); OASIS documentation familiarity preferred
- Strong assessment, organizational, and communication skills
- Reliable vehicle, valid drivers license, and proof of auto insurance
- Ability to manage a flexible field-based schedule independently
