Job Description

A rewarding opportunity awaits a skilled and compassionate Physical Therapist Assistant (PTA) with experience in home health care. In this field-based position, you will deliver personalized physical therapy interventions to patients in their homes, supporting improved mobility, independence, and overall quality of life. Working closely under the supervision of a licensed Physical Therapist, youll be instrumental in achieving optimal patient outcomes by following established plans of care and collaborating across an interdisciplinary team.

Schedule & Assignment Details:

  • Monday Friday, 8:00 AM 5:00 PM
  • Full-time, 13-week contract
  • Caseload: Approximately 2532 points weekly (57 patient visits per day, visit type dependent)
  • Requires daily travel within the assigned territory (mileage reimbursement provided)
  • Electronic documentation via Home Care Home Base (HCHB)

What youll do:

  • Deliver skilled therapy treatments, including therapeutic exercise, gait and balance training, and functional mobility tasks per the prescribed plan of care
  • Track, document, and report patient progress, noting changes in condition and collaborating closely with the supervising PT and other care team members
  • Reinforce patient and caregiver education on safety, assistive devices, and individualized home exercise programs
  • Maintain timely, accurate documentation in HCHB and participate in team case conferences
  • Abide by all agency policies and regulatory standards, including Medicare home health requirements

Required qualifications:

  • Current, active Physical Therapist Assistant (PTA) license (state-specific, in good standing)
  • Minimum of one year recent home health PTA experience; field work preferred
  • Proficiency with Home Care Home Base (HCHB) and OASIS documentation a plus
  • Effective communication, strong clinical observation, and organizational skills
  • Reliable vehicle, drivers license, and insurance
