Job Description

Deliver skilled and patient-focused care as a Home Health Physical Therapist Assistant (PTA) in a vital field-based role. This contract position plays an essential part in enhancing patients functional independence and quality of life. Work collaboratively with the supervising Physical Therapist to implement individualized treatment plansall while enjoying autonomy and a supportive team environment.

Schedule:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM
  • Caseload: 2532 points per week (approximately 57 patient visits/day)
  • Field-based; daily travel to patient homes within the assigned area

Qualifications:

  • Active and valid PTA license in the state
  • At least 1 year of recent experience in Home Health as a PTA (field experience preferred)
  • Proficiency with Home Care Home Base (HCHB) preferred
  • Familiarity with OASIS documentation processes is an advantage
  • Excellent clinical observation, time management, and organizational skills
  • Capable of working independently in patient homes
  • Strong communication skills with team members, patients, and caregivers
  • Reliable transportation, current drivers license, and auto insurance

Key Responsibilities:

  • Deliver physical therapy interventions as directed by the supervising PT, focused on exercises, gait training, transfer training, and functional mobility
  • Monitor and report patient responses, progress, and changes in condition to PT and care team
  • Provide patient and caregiver instruction on safety, assistive devices, and home exercise plans
  • Accurately maintain electronic patient documentation using HCHB
  • Collaborate in case conferences and interdisciplinary team meetings
  • Ensure compliance with all agency, medicare, and regulatory standards
