Job Description

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

In this 13-week contract role, you will work directly with patients in their homes, assisting with therapeutic exercises, gait and balance training, transfer activities, and overall mobility improvement. You will help implement individualized treatment plans, monitor patient progress, and maintain detailed documentation using the Home Care Home Base (HCHB) system. As a valued member of the care team, youll reinforce education for patients and caregivers regarding safety strategies, exercise regimens, and use of assistive devices.

Key Qualifications:

  • Active Physical Therapist Assistant (PTA) license in good standing for the state of practice
  • At least 1 year of recent home health PTA experience (field experience preferred)
  • Familiarity with Home Care Home Base (HCHB); OASIS knowledge is a plus
  • Excellent observation, communication, organization, and time management skills
  • Reliable transportation, valid drivers license, and automobile insurance
  • Ability to work independently across a flexible, field-based schedule

Your primary responsibilities will include collaborating with members of the interdisciplinary healthcare team, participating in case conferences, and ensuring compliance with agency policies and Medicare guidelines. Strong clinical judgment and organizational skills are essential for success in this role.
