Job Description
Deliver impactful, patient-centered therapy as a Home Health Physical Therapist Assistant (PTA) serving individuals in the comfort of their homes. Work closely with a supervising Physical Therapist to implement tailored treatment plans, foster patient independence, and contribute meaningfully to recovery journeys in a field-based environment.
Schedule & Work Structure:
- Monday to Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Daily travel within a defined territory (mileage reimbursement provided)
- Caseload: Approx. 2532 points weekly, typically 57 patient visits per day
Essential Responsibilities:
- Carry out therapeutic exercises, gait and balance training, and transfer activities following the supervising Physical Therapists plan.
- Monitor patient response, adapt interventions as directed, and document progress precisely using Home Care Home Base (HCHB).
- Communicate changes and updates to the supervising PT and interdisciplinary team.
- Educate patients and caregivers in safety techniques, the use of assistive devices, and at-home exercise protocols.
- Participate in interdisciplinary meetings and case conferences, ensuring cohesive care.
- Adhere consistently to agency policies, regulatory standards, and Medicare guidelines.
What Were Looking For:
- Active PTA license in good standing
- At least 1 year of recent home health experience in a similar setting
- Proficiency with Home Care Home Base (HCHB) preferred
- Familiarity with OASIS documentation considered an asset
- Strong interpersonal skills, self-motivation, and organization
- Capability to work independently and safely in patient homes
- Reliable transportation, a valid drivers license, and auto insurance
