Job Description

Deliver high-quality, patient-centered care as a Home Health Physical Therapist Assistant. In this vital role, you will provide skilled therapy services in patient homes, helping individuals regain mobility, strength, and independence under the guidance of a licensed Physical Therapist. Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Full-time, 13-week contract

Key Responsibilities:

  • Administer physical therapy treatments consistent with plans established by the supervising Physical Therapist.
  • Assist patients with therapeutic exercises, gait and balance training, transfers, and functional mobility activities.
  • Monitor and accurately document patient progress toward functional goals using Home Care Home Base (HCHB).
  • Communicate patient status and updates to the PT and interdisciplinary care team.
  • Reinforce patient and caregiver education on exercise techniques, safety protocols, fall prevention, and use of assistive devices.
  • Participate in case conferences and collaborate with nurses, occupational therapists, speech therapists, and social workers.
  • Uphold regulatory requirements, agency policies, and Medicare guidelines.

Qualifications:

  • Active Physical Therapist Assistant (PTA) license in good standing.
  • Minimum 1 year of recent home health PTA experience; field experience preferred.
  • Strong command of Home Care Home Base (HCHB) and familiarity with OASIS documentation preferred.
  • Excellent observation, communication, and organizational abilities.
  • Reliable transportation, a valid drivers license, and proof of auto insurance.
  • Ability to self-manage and adapt to a flexible field-based schedule.
