Step into a supportive environment as a Home Health Physical Therapist Assistant (PTA), delivering specialized care and therapy services directly to patients in their homes. This full-time, 13-week contract position offers the chance to build meaningful relationships and foster improved patient outcomes within the community.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Administer skilled physical therapy interventions as outlined by the supervising Physical Therapist.
• Guide patients through therapeutic exercises, gait and balance activities, transfer techniques, and mobility tasks.
• Track progress and document patient response to therapy using Home Care Home Base (HCHB).
• Clearly communicate patient status and any changes to the supervising PT and relevant care team members.
• Provide patient and caregiver education on safe home mobility strategies, assistive devices, fall prevention, and ongoing exercise programs.
• Actively participate in care team meetings and case conferences.
• Uphold all agency and regulatory protocols, including Medicare home health standards.

Desired Qualifications:

• Active Physical Therapist Assistant (PTA) license in good standing.
• At least 1 year of recent experience as a Home Health PTA, with field-based experience preferred.
• Familiarity with Home Care Home Base (HCHB) and OASIS documentation processes.
• Exceptional clinical observation, time management, and communication abilities.
• Reliable transportation, valid drivers license, and current auto insurance.
• Comfortable working independently in a flexible, field-based schedule.
