Dynamic opportunity available for an experienced Physical Therapist Assistant (PTA) to deliver exceptional care to patients in a home health setting. This contract role offers the chance to work independently, contributing to improved patient outcomes by implementing therapeutic interventions and fostering patient independence right at home.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Deliver skilled physical therapy in alignment with treatment plans developed by a licensed Physical Therapist.
- Support patients with therapeutic exercises, balance activities, functional mobility, transfer, and gait training.
- Document patient progress, monitor clinical outcomes, and communicate updates to the supervising PT and interdisciplinary team.
- Educate patients and caregivers on home exercise programs, safety strategies, fall prevention, and adaptive device use.
- Attend case conferences and collaborate with other healthcare professionals, including nurses and therapists.
- Utilize the Home Care Home Base (HCHB) system for timely, accurate electronic documentation.
- Maintain compliance with regulatory and safety standards as well as agency policies.
- Travel daily within the assigned territory to patient homes (mileage reimbursement provided).
Desired Qualifications:
- Current, active Physical Therapist Assistant (PTA) license in good standing for state of practice.
- Minimum one year of recent home health PTA experience required; field-based experience preferred.
- Proficiency with Home Care Home Base (HCHB) and familiarity with OASIS documentation highly preferred.
- Strong written and verbal communication, observation, and critical thinking skills.
- Valid drivers license, reliable vehicle, and auto insurance required.
- Self-motivated, highly organized, and adaptable to a flexible field-based caseload.
