Job Description
Bring your expertise as a Home Health Physical Therapist Assistant (PTA) to a fulfilling role centered on delivering patient-centered care to individuals in the comfort of their homes. In this full-time, field-based position, you will support patients in achieving greater mobility, strength, and independence under the supervision of a licensed Physical Therapist, implementing tailored treatment plans while working independently and collaboratively.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Responsibilities:
- Deliver skilled physical therapy treatments as directed by the supervising Physical Therapist, focusing on therapeutic exercises, mobility, and balance activities.
- Support patients with gait training, transfer techniques, fall prevention strategies, and functional mobility tasks.
- Monitor and document patient progress, updating the care team and ensuring seamless communication on patient status and treatment outcomes.
- Educate patients and caregivers on home exercise programs, safe practices, and the use of assistive devices to promote ongoing independence.
- Maintain thorough, timely documentation within Home Care Home Base (HCHB).
- Collaborate with interdisciplinary team members, including other therapists, nurses, and home health aides.
- Participate in team meetings and adhere to agency policies and home health regulations.
Qualifications:
- Active Physical Therapist Assistant (PTA) license in good standing
- Minimum of one year of recent field-based Home Health PTA experience
- Proficiency in Home Care Home Base (HCHB) is highly preferred
- Understanding of OASIS documentation is an asset
- Strong communication, organization, and independent work skills
- Reliable transportation, valid drivers license, and active auto insurance
