Make an impact in patient care as a Home Health Physical Therapist Assistant (PTA), providing skilled therapy services directly in patients homes under the guidance of a licensed Physical Therapist. This contract opportunity empowers you to deliver individualized care designed to maximize mobility, independence, and quality of life for clients in a rewarding, field-based environment.
Schedule & Details:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
- Caseload of approximately 2532 points per week (57 visits daily)
- Field-based, visiting patient homes; daily travel within assigned territory (mileage reimbursement provided)
- Utilization of electronic documentation system: Home Care Home Base (HCHB)
Key Responsibilities:
- Administer skilled physical therapy treatments as directed by the supervising Physical Therapist, helping patients achieve functional goals.
- Assist with therapeutic exercises, gait and balance training, transfer activities, and mobility interventions.
- Monitor, document, and report patient response, condition changes, and progress to the supervising PT and care team.
- Educate patients and caregivers on exercises, safety strategies, proper device use, and fall prevention.
- Maintain thorough and timely documentation within HCHB.
- Collaborate with interdisciplinary teams during case conferences and meetings to support comprehensive care.
- Uphold agency policies, home health regulations, and Medicare standards.
Required Qualifications:
- Active Physical Therapist Assistant (PTA) license in good standing.
- At least 1 year recent Home Health PTA experience (field experience preferred).
- Familiarity with Home Care Home Base (HCHB) and OASIS processes preferred.
- Effective communication, strong clinical evaluation, and organizational skills.
- Reliable transportation, active drivers license, and current automobile insurance.
- Ability to manage a flexible, autonomous field schedule.
