Elevate your career by providing compassionate care as a Home Health Physical Therapist Assistant (PTA), supporting patients as they regain independence in the comfort of their own homes. Working under the supervision of a licensed Physical Therapist, you will play a key role in delivering patient-centered therapy, carrying out individualized treatment plans focused on improving mobility, strength, and daily function.
Schedule:
- Full-time, 13-week contract
- Monday to Friday, 8:00 AM 5:00 PM
- Expect a caseload of approximately 2532 points per week (typically 57 patient visits daily)
- Field-based, with daily travel to patient homes (mileage reimbursement provided)
Key Responsibilities:
- Deliver skilled therapy interventions as directed by the supervising Physical Therapist.
- Assist with prescribed exercises, gait and balance training, transfer techniques, and functional mobility tasks.
- Monitor and document patient response and progress using the Home Care Home Base (HCHB) system.
- Report significant changes in the patients condition to the supervising PT and care team.
- Educate patients and caregivers on safe techniques, use of assistive devices, and home exercise regimens.
- Maintain timely and accurate electronic documentation.
- Collaborate with a multidisciplinary healthcare team; attend case conferences and team meetings.
- Uphold agency policies and comply with Medicare and regulatory standards.
Preferred Qualifications:
- Active PTA license in good standing in the state of practice
- At least 1 year of recent Home Health PTA experience
- Familiarity with Home Care Home Base (HCHB) and OASIS documentation preferred
- Excellent clinical observation, organizational, and communication skills
- Reliable transportation, valid drivers license, and auto insurance
