Elevate your career by providing compassionate care as a Home Health Physical Therapist Assistant (PTA), supporting patients as they regain independence in the comfort of their own homes. Working under the supervision of a licensed Physical Therapist, you will play a key role in delivering patient-centered therapy, carrying out individualized treatment plans focused on improving mobility, strength, and daily function.

Schedule:

• Full-time, 13-week contract
• Monday to Friday, 8:00 AM 5:00 PM
• Expect a caseload of approximately 2532 points per week (typically 57 patient visits daily)
• Field-based, with daily travel to patient homes (mileage reimbursement provided)

Key Responsibilities:

• Deliver skilled therapy interventions as directed by the supervising Physical Therapist.
• Assist with prescribed exercises, gait and balance training, transfer techniques, and functional mobility tasks.
• Monitor and document patient response and progress using the Home Care Home Base (HCHB) system.
• Report significant changes in the patients condition to the supervising PT and care team.
• Educate patients and caregivers on safe techniques, use of assistive devices, and home exercise regimens.
• Maintain timely and accurate electronic documentation.
• Collaborate with a multidisciplinary healthcare team; attend case conferences and team meetings.
• Uphold agency policies and comply with Medicare and regulatory standards.

Preferred Qualifications:

• Active PTA license in good standing in the state of practice
• At least 1 year of recent Home Health PTA experience
• Familiarity with Home Care Home Base (HCHB) and OASIS documentation preferred
• Excellent clinical observation, organizational, and communication skills
• Reliable transportation, valid drivers license, and auto insurance
