Job Description

A skilled Home Health Physical Therapist Assistant (PTA) is needed to provide compassionate and goal-oriented therapy services in patients homes under the guidance of a licensed Physical Therapist. This contract opportunity is ideal for candidates who thrive in autonomous, field-based roles and are driven to deliver exceptional patient-centered care. The successful PTA will implement personalized treatment plans, support patients in regaining functional independence, and play a pivotal role in ensuring positive therapy outcomes.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Full-time, 13-week contract assignment

Desired Qualifications:

  • Current and active Physical Therapist Assistant (PTA) license, in good standing
  • At least 1 year of recent home health PTA experience; strong field-based background is a plus
  • Familiarity with Home Care Home Base (HCHB) electronic documentation system preferred
  • Understanding of OASIS documentation processes is beneficial
  • Excellent clinical observation, communication, and patient engagement skills
  • Reliable transportation, valid drivers license, and auto insurance required
  • Strong time management, organization, and independence in a mobile role

Key Responsibilities:

  • Deliver skilled therapy interventions based on individualized plans of care set by the supervising PT
  • Provide therapeutic exercises, gait and balance activities, and transfer training to improve patient outcomes
  • Monitor and record patient progress, promptly communicating changes to the treatment team
  • Educate patients and caregivers on exercises, home safety, fall prevention, and the proper use of adaptive equipment
  • Accurately complete documentation in Home Care Home Base (HCHB) system
  • Collaborate with interdisciplinary team members and participate in case conferences as needed
  • Follow all policies, procedures, and Medicare guidelines for home health agencies
