Job Description
Take on a rewarding role delivering compassionate care as a Home Health Physical Therapist Assistant (PTA), working collaboratively in a field-based setting to help patients regain independence and function in their own homes. You'll work under the supervision of a licensed Physical Therapist, implementing tailored treatment plans that improve mobility, strength, and balance, and making a meaningful difference in patients' daily lives.
Schedule:
- Full-time, 13-week contract
- Monday Friday, 8:00 AM 5:00 PM
Key Duties:
- Provide skilled therapy in accordance with plans established by the supervising PT.
- Guide patients through therapeutic exercises, gait and balance training, transfer techniques, and functional mobility interventions.
- Monitor and document patient responses, communicating progress and changes to the Physical Therapist and interdisciplinary care team.
- Educate patients and caregivers on home exercise programs, safety, assistive devices, and fall prevention.
- Use Home Care Home Base (HCHB) for accurate, timely documentation.
- Collaborate regularly with nurses, therapy staff, social workers, and other professionals to coordinate care.
- Participate in case conferences as needed and adhere to all relevant healthcare and Medicare regulations.
Qualifications:
- Active PTA license in good standing in the state of practice
- At least 1 year of recent field-based Home Health PTA experience preferred
- Familiarity with Home Care Home Base (HCHB) and OASIS documentation processes preferred
- Reliable transportation, drivers license, and auto insurance required
- Excellent clinical, time management, and communication abilities
- Highly motivated and able to work independently
