Job Description
Deliver exceptional care and support to patients in their homes by joining a leading team of healthcare professionals as a Home Health Physical Therapist Assistant (PTA). This rewarding role is ideal for compassionate, motivated individuals who thrive working independently and value meaningful patient interaction in a field-based setting.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Daily travel to patient homes within assigned territory (mileage reimbursement provided)
- Caseload: Approximately 2532 points per week (typically 57 patient visits per day)
- Utilize the Home Care Home Base (HCHB) electronic documentation system
Qualifications:
- Active Physical Therapist Assistant (PTA) license in good standing
- Minimum of 1 year of recent Home Health experience as a PTA, with field-based experience strongly preferred
- Familiarity with Home Care Home Base (HCHB) and understanding of OASIS documentation processes preferred
- Reliable transportation, valid drivers license, and auto insurance
- Strong clinical observation, communication, and organizational skills
- Comfortable working autonomously and managing a flexible schedule
Key Responsibilities:
- Deliver skilled physical therapy interventions as outlined by the supervising Physical Therapist
- Assist patients with therapeutic exercises, gait and balance training, transfer techniques, and activities for improved mobility and independence
- Accurately monitor, document, and communicate patient progress and changes to the clinical team
- Educate patients and caregivers on safety, assistive device use, and customized home exercise programs
- Collaborate with an interdisciplinary team including nurses, occupational therapists, speech-language pathologists, and social workers
- Adhere to all agency, regulatory, and Medicare guidelines
