Job Description
Deliver compassionate, impactful care as a Home Health Physical Therapist Assistant (PTA) supporting patients in their recovery across community settings. As a valued member of the therapy team, youll play a vital role by working directly with individuals in their homesensuring they regain strength, improve mobility, and maximize independence under the supervision of a licensed Physical Therapist.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- 13-week, full-time contract assignment
- Caseload of approximately 2532 points weekly (57 visits per day, based on visit types)
Key Responsibilities:
- Provide hands-on physical therapy treatments aligned with the supervising Physical Therapists care plan.
- Guide patients through exercises, gait, balance, transfer techniques, and functional mobility tasks.
- Observe and document patient progress, reporting changes promptly to the PT and care team.
- Educate patients and caregivers on safety strategies, proper use of assistive devices, fall prevention, and personalized home exercise routines.
- Maintain accurate documentation using the Home Care Home Base (HCHB) system.
- Collaborate with other healthcare professionalsincluding nurses, OTs, SLPs, social workers, and home health aidesto optimize patient outcomes.
- Participate actively in case conferences and team meetings, following agency and regulatory guidelines.
- Conduct daily travel to patient homes (mileage reimbursement included).
Qualifications:
- Current, active PTA license in state of practice (in good standing).
- At least 1 year of recent home health PTA experience; field-based experience preferred.
- Familiarity with Home Care Home Base (HCHB) and OASIS documentation processes is a plus.
- Strong clinical observation, time management, and organizational skills.
- Reliable transportation, valid drivers license, and auto insurance required.
- Ability to work independently in a field-based role.
