A compassionate and self-motivated Physical Therapist Assistant (PTA) is needed to deliver skilled therapy services to patients in their homes. In this multi-faceted role, you'll implement individualized care plans under a Physical Therapists guidance, assisting patients to regain mobility, strength, and independence in the comfort of their own environment.

Schedule and Details:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• 2532 points per week (typical caseload: 57 home visits daily)
• Daily travel within a defined territory; mileage reimbursement provided
• Documentation via Home Care Home Base (HCHB)

Key Responsibilities:

• Administer physical therapy interventions as prescribed, promoting patient safety and progression.
• Guide patients through therapeutic exercises, gait and transfer training, balance activities, and functional mobility.
• Monitor and document clinical progress and patient responses; communicate updates with supervising PT and care team.
• Educate patients and caregivers on exercise programs, safety, and use of assistive devices to foster recovery.
• Maintain up-to-date, accurate records in HCHB.
• Participate in interdisciplinary team conferences and collaborate with other healthcare professionals.
• Ensure all clinical and agency policies, Medicare, and regulatory standards are met.

Required Qualifications:

• Active, good-standing Physical Therapist Assistant license for the state of practice.
• At least one year of recent experience as a Home Health PTA (field-based experience strongly preferred).
• Familiarity with Home Care Home Base (HCHB) and OASIS documentation preferred.
• Excellent clinical, organizational, and communication skills.
• Reliable transportation, drivers license, and auto insurance required.
• Ability to work independently and prioritize a flexible field schedule.
