Job Description
Deliver vital care and support patients mobility and independence as a Home Health Physical Therapist Assistant (PTA). This full-time, 13-week contract is centered around providing high-quality, patient-centered physical therapy services directly in patients homes. The role is ideal for individuals who are self-driven, organized, and comfortable working independently while collaborating with a multidisciplinary team to ensure positive patient outcomes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Qualifications:
- Active Physical Therapist Assistant (PTA) license in good standing
- At least 1 year of recent Home Health PTA experience (field-based experience preferred)
- Familiarity with Home Care Home Base (HCHB) electronic documentation system preferred
- Understanding of OASIS documentation processes (preferred)
- Reliable transportation, valid drivers license, and current auto insurance
- Strong clinical observation, communication, and organizational abilities
- Capacity to work autonomously in a flexible, field-based environment
Core Responsibilities:
- Implement skilled physical therapy interventions as outlined by the supervising Physical Therapist
- Guide patients through therapeutic exercises, gait and transfer training, balance work, and other mobility-enhancing tasks
- Monitor and document patient responses and progress in Home Care Home Base (HCHB)
- Communicate changes in patient status to the supervising PT and team members
- Provide education to patients and caregivers on home exercises, safety measures, and use of assistive devices
- Collaborate with the broader interdisciplinary team and participate in case conferences as needed
- Maintain compliance with all relevant policies, regulations, and Medicare home health standards
