Job Description

A skilled and compassionate Home Health Physical Therapist Assistant is needed to deliver high-quality therapy services to patients in their homes. Working under the supervision of a licensed Physical Therapist, you will be responsible for supporting patients functional recovery, implementing customized treatment plans, and fostering independence and safety. This is a full-time, 13-week contract position, offering the opportunity to make a meaningful impact while working independently in a dynamic, field-based setting.

Schedule and Hours:

  • Monday Friday, 8:00 AM 5:00 PM
  • Field-based; daily travel to patient homes within the assigned territory
  • Caseload: Approximately 2532 points per week (about 57 patient visits per day, depending on visit type)

Key Responsibilities:

  • Deliver skilled physical therapy interventions in accordance with the plan of care developed by the supervising PT
  • Assist patients with exercises, transfer training, balance, gait training, and functional mobility tasks
  • Monitor and document patient responses and progress toward rehabilitation goals
  • Communicate changes in patient status to the PT and other care team members
  • Reinforce patient and caregiver education on safety, equipment, and home exercise routines
  • Utilize Home Care Home Base (HCHB) for accurate and timely documentation
  • Collaborate with the interdisciplinary team and take part in meetings and case conferences
  • Adhere to agency policies, regulatory standards, and Medicare home health guidelines

Qualifications:

  • Active Physical Therapist Assistant license (in good standing)
  • At least 1 year of recent Home Health PTA experience preferred
  • Proficiency in Home Care Home Base (HCHB) preferred
  • Familiarity with OASIS documentation a plus
  • Reliable transportation, valid drivers license, and current auto insurance
  • Exceptional organizational, communication, and time management abilities
