Job Description

A rewarding opportunity is available for a skilled Home Health Physical Therapist Assistant (PTA) eager to make a direct impact on patients daily lives. This contract role focuses on delivering high-quality therapy to individuals in their own homes, working closely under the supervision of a licensed Physical Therapist. Youll play a vital role in helping patients regain strength, mobility, and independence, with autonomy and flexibility in the field.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM

Core Qualifications:

  • Active, unrestricted Physical Therapist Assistant (PTA) license in the state of practice
  • At least 1 year of recent experience as a PTA in home health (field/mobile setting preferred)
  • Working knowledge of Home Care Home Base (HCHB) preferred
  • Familiarity with OASIS documentation processes preferred
  • Strong clinical assessment, documentation, and patient communication abilities
  • Reliable transportation, valid drivers license, and active auto insurance
  • Ability to work independently and manage a flexible, field-based workload

Key Responsibilities:

  • Deliver skilled physical therapy interventions outlined by the supervising Physical Therapist
  • Assist patients with exercises, transfer training, balance/gait activities, and activities of daily living
  • Monitor, report, and document patient progress toward individualized goals
  • Communicate changes in patient status to the supervising PT and interdisciplinary team
  • Reinforce home exercise and safety education for patients and caregivers
  • Maintain accurate and timely documentation using HCHB
  • Attend case conferences and team meetings as needed
  • Meet all regulatory, Medicare, and agency compliance guidelines
