Job Description

A skilled Home Health Physical Therapist Assistant (PTA) is needed to deliver compassionate, patient-centered therapy services in a field-based role. Working under the supervision of a licensed Physical Therapist, youll provide essential care that helps patients regain mobility, improve balance and strength, and achieve greater independenceall from the comfort of their homes.

Schedule:

  • Full-time, 13-week contract
  • Monday Friday, 8:00 AM 5:00 PM
  • Approximately 2532 points per week (typically 57 patient visits/day depending on visit types)

Key Responsibilities:

  • Implement physical therapy interventions as outlined by the supervising PT
  • Guide patients through therapeutic exercises, gait and balance activities, transfer training, and functional mobility routines
  • Track and document patient progress, communicating meaningful updates or changes to the supervising therapist
  • Reinforce patient and caregiver education about safety, the use of assistive devices, fall prevention, and home exercise programs
  • Maintain accurate documentation using Home Care Home Base (HCHB)
  • Collaborate with the broader home health team by participating in case conferences and regular meetings
  • Ensure compliance with agency, regulatory, and Medicare guidelines

Requirements:

  • Active PTA license in good standing within state of practice
  • At least 1 year of recent home health PTA experience (field experience preferred)
  • Familiarity with Home Care Home Base (HCHB) strongly desired
  • Understanding of OASIS documentation is a plus
  • Excellent observation, documentation, and communication skills
  • Reliable transportation, a valid drivers license, and appropriate insurance
  • Ability to work independently and effectively manage a field-based schedule
