A fulfilling opportunity is available for a skilled and compassionate Physical Therapist Assistant (PTA) to provide vital therapy services in a home health setting. Supporting patients in their own homes under the direction of a licensed Physical Therapist, you will play a crucial role in helping individuals regain and maintain their independence and mobility. This role requires a self-directed professional who is comfortable working independently in a dynamic, field-based environment.

Schedule & Employment Details:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Estimated caseload: 2532 points per week (generally 57 patient visits per day)
• Field-based setting with daily travel in an assigned territory (mileage reimbursement included)
• Utilization of the Home Care Home Base (HCHB) electronic documentation system

Key Responsibilities:

• Deliver skilled physical therapy interventions following customized care plans established by the supervising PT
• Instruct and assist patients with exercises, functional mobility, transfer training, and gait/balance activities
• Monitor and accurately document patients progress and communicate changes to the care team
• Educate patients and caregivers on safety, fall prevention, and home exercise programs
• Ensure consistent, thorough, and timely documentation in HCHB
• Collaborate with a diverse interdisciplinary healthcare team
• Comply with all regulatory and agency standards, including Medicare guidelines

Qualifications:

• Current, active PTA license in good standing for the state in which you will practice
• At least one year of recent experience as a PTA in a home health setting preferred
• Proficiency with HCHB and understanding of OASIS processes is a plus
• Excellent assessment, communication, and time management skills
• Valid drivers license and reliable transportation required
