A fulfilling opportunity is available for a skilled and compassionate Physical Therapist Assistant (PTA) to provide vital therapy services in a home health setting. Supporting patients in their own homes under the direction of a licensed Physical Therapist, you will play a crucial role in helping individuals regain and maintain their independence and mobility. This role requires a self-directed professional who is comfortable working independently in a dynamic, field-based environment.
Schedule & Employment Details:
- Monday Friday, 8:00 AM 5:00 PM
- Full-time, 13-week contract
- Estimated caseload: 2532 points per week (generally 57 patient visits per day)
- Field-based setting with daily travel in an assigned territory (mileage reimbursement included)
- Utilization of the Home Care Home Base (HCHB) electronic documentation system
Key Responsibilities:
- Deliver skilled physical therapy interventions following customized care plans established by the supervising PT
- Instruct and assist patients with exercises, functional mobility, transfer training, and gait/balance activities
- Monitor and accurately document patients progress and communicate changes to the care team
- Educate patients and caregivers on safety, fall prevention, and home exercise programs
- Ensure consistent, thorough, and timely documentation in HCHB
- Collaborate with a diverse interdisciplinary healthcare team
- Comply with all regulatory and agency standards, including Medicare guidelines
Qualifications:
- Current, active PTA license in good standing for the state in which you will practice
- At least one year of recent experience as a PTA in a home health setting preferred
- Proficiency with HCHB and understanding of OASIS processes is a plus
- Excellent assessment, communication, and time management skills
- Valid drivers license and reliable transportation required
