A fulfilling opportunity awaits for a licensed Physical Therapist Assistant (PTA) to deliver in-home therapy services for adult patients. This position centers on compassionate care and patient empowerment, enabling individuals to regain independence within the comfort of their own homes. Under the supervision of a Physical Therapist, youll help implement tailored therapy plans, monitor patient response, and support families with ongoing education.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- Full-time, 13-week assignment
Desired Qualifications:
- Active PTA license in the state of practice, in good standing
- Minimum one year of recent experience providing Home Health PTA services (field-based experience preferred)
- Working knowledge of Home Care Home Base (HCHB) system preferred
- Familiarity with OASIS documentation processes preferred
- Strong clinical observation and communication skills
- Reliable transportation, valid drivers license, and active auto insurance
- Able to work independently, demonstrating excellent organizational and time management skills
Key Responsibilities:
- Deliver prescribed physical therapy interventions including therapeutic exercises, gait and balance training, and transfer assistance under PT direction
- Accurately monitor, document, and report on patient progress to supervising PT and care team
- Educate patients and caregivers on safety, assistive devices, fall prevention, and home exercise routines
- Maintain timely and detailed electronic documentation in HCHB
- Engage in interdisciplinary collaboration, including attendance at case conferences and team meetings
- Ensure practices comply with agency policies and Medicare home health guidelines
- Manage a flexible schedule, traveling daily to patients homes within the assigned territory (mileage reimbursement provided)
