Job Description
Deliver expert care and help patients regain their independence as a Home Health Physical Therapist Assistant (PTA) on a full-time, 13-week contract. In this rewarding field-based position, you will provide individualized physical therapy interventions to patients in their homes, collaborating closely with a licensed Physical Therapist to promote mobility, safety, and functional outcomes.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
Key Qualifications:
- Active Physical Therapist Assistant (PTA) license in good standing
- At least one year of recent home health PTA experience (field-based experience preferred)
- Familiarity with Home Care Home Base (HCHB) electronic documentation highly desirable
- Knowledge of OASIS documentation processes is a plus
- Excellent clinical, observational, and communication abilities
- Strong organization and time management skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently, efficiently managing a flexible field-based schedule
Primary Responsibilities:
- Deliver skilled physical therapy treatments based on the PTs plan of care
- Assist patients with exercises, gait and balance activities, transfer training, and functional mobility tasks
- Accurately monitor, document, and report on patient status, progress, and care needs
- Reinforce and educate patients and caregivers on safe techniques and individualized home exercise programs
- Maintain timely, precise records using HCHB
- Work collaboratively with the interdisciplinary healthcare team
- Participate in case conferences and team meetings as needed
- Uphold agency procedures and comply with regulatory standards
