Qualifications
- Active North Carolina PT license
- Experience in Home Health PT, Oasis, and HCHB systems
- Adaptability to diverse patient cases and county-wide travel
Responsibilities
- Conduct comprehensive physical therapy evaluations based on physician referrals
- Develop individualized treatment plans with measurable goals for increased independence and function
- Continuously reassess patient progress, updating goals and plans as necessary
- Communicate patient progress and needs to the treatment team, families, and medical providers
- Provide equipment recommendations and incorporate adaptive tools into therapy
- Maintain accurate, timely documentation according to all regulatory and company standards
- Supervise Physical Therapist Assistants and Aides in line with state requirements
- Participate in staff development, meetings, and ongoing clinical education

$2,011 - $2,127/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
