Schedule:
- Monday through Friday (No on-call)
Key Qualifications:
- Active North Carolina PT license required
- Experience in Home Health PT, Oasis, HCHB
- Familiarity with physical therapy evaluations, developing care plans, and adaptive equipment
- Capable of reviewing patient records for diagnosis/history
- Ability to supervise Physical Therapist Assistants and Aides
Responsibilities:
- Perform patient evaluations and treatments as prescribed
- Complete comprehensive assessments and timely documentation
- Develop measurable goals and adapt care plans based on patient progress
- Communicate with physicians, family, and interdisciplinary team
- Recommend/adapt special equipment to enhance patient function
- Maintain compliance with all regulations, codes, and corporate policies
- Participate in staff development and continuing education
