Schedule:

  • Monday through Friday (No on-call)

Key Qualifications:

  • Active North Carolina PT license required
  • Experience in Home Health PT, Oasis, HCHB
  • Familiarity with physical therapy evaluations, developing care plans, and adaptive equipment
  • Capable of reviewing patient records for diagnosis/history
  • Ability to supervise Physical Therapist Assistants and Aides

Responsibilities:

  • Perform patient evaluations and treatments as prescribed
  • Complete comprehensive assessments and timely documentation
  • Develop measurable goals and adapt care plans based on patient progress
  • Communicate with physicians, family, and interdisciplinary team
  • Recommend/adapt special equipment to enhance patient function
  • Maintain compliance with all regulations, codes, and corporate policies
  • Participate in staff development and continuing education
